I voluntarily consent to being recorded for The Colorado Psychedelic Church Podcast. I understand the recording may include my voice, image, likeness, name, and statements, and may be audio, video, or both.

I grant The Colorado Psychedelic Church and its representatives permission to record, edit, reproduce, publish, distribute, display, promote, and otherwise use the recording, in whole or in part, in any media or format, including podcast platforms, websites, social media, video platforms, marketing materials, educational materials, and archival use.

Name Preference

I understand I may request to be identified by a preferred name or pseudonym instead of my legal name.

No Compensation

I understand that my participation is voluntary and that I will not receive compensation, royalties, or payment unless otherwise agreed in writing.

No Professional Advice

I understand that The Colorado Psychedelic Church Podcast is for educational, informational, spiritual, and conversational purposes only. The podcast does not provide medical, legal, therapeutic, psychological, or financial advice.

Psychedelic Experiences and Personal Disclosures

I understand that the conversation may include topics related to psychedelics, spirituality, wellness, personal experiences, or health-related matters. I voluntarily choose what information to share and understand that I am responsible for my own statements and disclosures.

Release of Claims

To the fullest extent permitted by law, I release and hold harmless The Colorado Psychedelic Church, its officers, volunteers, agents, representatives, and podcast team from claims arising from the recording or use of the content as described in this agreement.
